Elective in General Hospital Sibu


this is me at A & E, got nothing to do, posing. XD hey, this is my first photo taken in labcoat. wohoo. though, the labcoat was borrowed from my friend's father's shop. a little too big for me. amniotic fluid, this color signified that the baby's in fetus distress. (If I'm not wrong)

Alright, I'm finally finished with my report. Later in the afternoon, I'm going to meet my new mentor to pass it up. What's written in the report were was happened during the elective, but not everything was recorded in it. haha.

It was fun, truly. I get to know new friend, a crazy friend. Lol... other than that, I got to witness many cases, patients, procedures. We were even offered to do the IV drip. The time we stayed was from monday to friday, from 8 till 5. But because I don't have the transportation, I had to stay until 6/7 for the first week. Lots of Unimas students who's also on elective come into the A & E department only after 5. The nurses said that there were lots more of patients and busier after 5.
I also see that how the doctors there were mostly MO and HO. Due to the fact that they are on call overnight at that department, they don't really have the enthusiasm to teach us. (not all, Dr Jesse was the best we met.)

At the O &G department, we got to witness baby birth. Aw... the baby was so cute. Really, I saw countless of baby delivered over the week. Some mothers took lots of time but some fast. I saw one of them, who took like... one hour for the delivery. She was an obese chinese. And she really took a lot of effort and was constantly at the point of giving up during the process. Okay, this is the bad part. At first, the nurses were advising and encouraging her nicely to push, teaching her the correct technique. But the mother couldn't get the correct method, and results in the baby couldn't reach the birth canal after half an hour. In the end, the nurses were becoming more impatient. I can't blame them because what they said was true. The baby was stuck in there for too long, no oxygen, it's bad for the health of the baby. The mother just had to suffer for a few minute, but if it delays, the baby might suffer mental problem for the rest of her life. Yes, it's a she. In the end, she managed to do it. luckily, phew.

on a side note, I kind of understand why M'sia government hospital lacked doctors. Look at their working schedule. Their working time for a HO were from 7 in the morning until 11/12 at night for the first 10 days at the new department. and then, the on call time will depend on number of people available in that department. Most of the things were left for the HO to do, and during on call, if there's emergency especially at the medical ward, I heard that it's the HO that'll be wakened up first. The HO has to solve the problem by him/herself before you can call up the MO. Ma, I guess, I'll worry about these later in my life.

No Sicko again ?

Starting from Jan this year until now, I had had a total of three times of flu. Considered quite often for a person of my age, huh? It was not like this, I remembered I rarely get sick during my college year, and back then, unlike now, I had no mother to 'mother' me by my side. And now, I'm feeling sore throat again. Should I expect another one coming in close future? Please... no.... I really hate it. >.<

How come it's different now? Isn't it supposed to be the other way round? :sigh:

RIPE

RIPE as in rifampicin, isoniazid, pyrazinamide, and ethambutol are drugs (first line) for tuberculosis.

Isoniazid, toxicity-- hepatotoxicity, bone marrow suppression. It reacts with pyridoxal to form complex that is easily excreted out the urine. As pyridoxal is important for the nutrition of nerve, low in pyridoxal after excretion cause peripheral neuritis, and convulsion, especially in malnourished people.

Rifampicin toxicity -- pseudomonas coliitis, CNS disturbancces (thus causing dizziness, confusion), liver damage, nausea, rashes. Give orange tinge to saliva. Also induces liver enzymes -- thus more degradation of for pts on warfarin, glucocorticoids, oral antidiabetics, i.e. less effective for these drugs to work.

Pyrazinamide toxicity -- hepatotoxicity. inhibits renal excretion of uric acid -- thus causing hyperuricaemia.

Ethambutol toxicity -- optic neuritis (blurred vision, green/red color blindness), GIT disturbances, headache, giddiness (huh? XD).

SECOND LINE ANTI TB
PAS (para-aminosalicylic acid)
toxicity -- epigastric pain, hypersensitivity reactions (fever, joint pains, rashes.)

Ciprofloxacin For NON - TB induced TB (atypical TB)
Ofloxacin for Mycobacterium tuberculosis induced. TB.

Sigh... I so wished the exam would finish. Studying could be so painful some times especially if you have two full stack of papers (each measured approximately 4cm height) to read, without knowing the border of the exam questions... It was so different from my high/college school, where we could estimate what questions would come out in the exam by doing lots of past year questions... Back then, all my 'As' are taken for granted.

But now, there's nothing like this. >.<

Kuala Kubu Bahru

two days without yaoi. XD I already felt really disconnected from my life just two or three days ago. Even though I heard how horrible the hostel is, I never expected it to be this worst.

The room, accomodated eight people, with four double decker beds. It was fine at first, but as night grows on, it get hotter and hotter. THe air that is circulating inside couldn't get out from the room, the result? It's stuffy and hot. And all thorough the night, I didn't sleep...
Second worst, No water. NO kettle. The water I tried to boil from my friend's kettle but the water stink. I had to buy the bottles with RM2 every day. -- money wasting.
Third worst, No food. The mamak and canteen doesn't suit my taste. while the town needed a fifteen min walk.
Fourth worst, need to stay over during weekend. Our group is the only group that needed to stay over for six days five nights. And it's only three weeks left till final n I still hadn't started to study seriously!

I came back by a friend's car, I am really grateful for that, but I still need to get back there tomo morning.. that still means my heart is... feeling very blue. I know I'm fussy. I know I'm complainiing... I know I'm selfish. I know I'm perssimistic. I know.

But I can't change that.
